Emergency Leave for Dependents Form

Employees have the right to time off to care for dependents in an emergency. This form should be completed by the employee on his/her return to work.

This will provide a record of the absence, ensure that the employee has not abused the right and allow the company to make a reasoned decision when deciding whether or not to authorise the absence.
