Authorised Absence Request

This form is for employees to use when requesting absence from work in situations where they have the right to request time off, or may want to for personal reasons. This would include where an employee requires time off for public duties, compassionate leave or to attend an appointment.

By using this form, your managers will be able to ensure the company meets its legal obligations, treats employees fairly and consistently, and yet be able to avoid abuse of the system by employees.
